3/9/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Jahsim upon arrival to the home setting. Upon arrival to the home setting, Jahsim appeared to be in a good mood. QMHP-C engaged Jahsim as well as his mother in a discussion to determine his behaviors since the last session. She reported that he continues to struggle with being able to control his anger when he cannot have his way. 2. Behavior: Jahsim was cooperative for the remainder of the session as he engaged in the session needing redirection from QMHP-C as he was hitting his brother when he walked by the room at the time of the session as what he appeared to be in angry because his brother was able to have a treat and he was not due to his behaviors within the home and school setting. 1.Intervention: QMHP-C engaged Jahsim in a therapeutic activity to promote healthier lifestyle and reinforce routine for reaching goals as stated by Jahsim. QMHP-C processed on problem solving and conflict resolutions to reduce disruptive behaviors. 2. Intervention: QMHP-C reviewed development of coping skills to reduce anger in the home and community and engaged in role play activity to practice interactions for modeling desired behaviors. QMHP-C discussed with Jahsim the information discussed within the therapeutic activity to provide framework for viewing conflict and relationship development and provided Jahsim with material for reading in preparation for further discussions. 1. Response: Jahsim was receptive in discussing how to reevaluate his responses when he becomes angry to help him settle his thoughts and feelings. Jahsim talked about angry outbursts making him feel better but negatively affects his relationship with his mother. 2. Response: Jahsim vented his emotions and discusses thoughts regarding his and feelings towards his family members guided by QMHP-C. QMHP-C verbalized his challenges with controlling his anger effectively and stated that he really wants to change.
